Funeral Application

Aboriginal Affairs NSW can make a contribution towards funeral costs. Please use the link below to download the application form. When applying for a Discretionary Funeral Grant please complete the application and attach the following documentation:

Confirmation of Aboriginality:

Only Aboriginal and Torres Strait Islander people who are residents of NSW will be considered for funding;
The deceased or the spouse of the deceased person must be of Aboriginal/Torres Strait Islander descent;
If the deceased person is not of Aboriginal/Torres Strait Islander descent, then his/her spouse must be Aboriginal and Torres Strait Islander in order to be considered for funding;
If the deceased person is not Aboriginal and/or Torres Strait Islander, evidence of an ongoing relationship with an Aboriginal and Torres Strait Islander spouse must be provided.

• Tax Invoice - Funeral Directors, and
• Death Certificate.
